In an effort to make your visits as straightforward and efficient as possible, we have compiled the most pertinent information for our patients to browse before they visit our office. Your First Visit

If you are visiting our office for the first time, we look forward to meeting you and welcoming you to the TTX PRIMARY CARE family. To streamline your first visit, you are welcome to print and fill out your new patient paperwork or submit your information virtually in the comfort and convenience of your own home. If you would prefer to fill out your paperwork in our office, please plan to arrive an additional 30 minutes early to your appointment, and we will be happy to provide you with the appropriate forms to fill out.

Medications & Previous Records

To create a comprehensive treatment plan that meets your needs, we welcome and encourage patients to provide us with any previous medical records that will provide our team with better insight into their unique situation. This can be done by requesting that your medical records be faxed to our office from a different practice (512-615-5188), or you may find it more convenient to print your records and bring them with you to your first visit. Please bring any medications that you are currently taking as well.

Cancellation Policy

If you can no longer attend an appointment for any reason, please provide our office with at least 24 hours’ notice if possible. This allows our team to see more patients and provide high-quality care promptly. We appreciate your understanding.
